forms.CheckboxInput is a widget – not a form field.

I think what you’re looking for is a forms.BooleanField

class MyForm(forms.Form):
    name = forms.CharField(label='Organization name')
    email = forms.EmailField(label='Organization email')
    trial_period = forms.BooleanField()
